ESA Form Tip: Side Effects On Meds

I found this on Fightback, who found it on ATOS Miracles on Monday.

Ive posted this before but another person has fell for the same trick: the ESA form does ask about side effects on meds, but its done very subtly, there is a question that asks if you suffer from alcohol or drug related problem and in small writing just above the answer box it states this also includes meds prescribed by a doctor. It is in this box all side effects of any meds should be written – its a bad way of writing it but it is there xxxxx

    You are quite right … I have never noticed the small writing and just ignored the box, thinking it was meant for people suffering from addiction to alcohol or creative drugs.

    No, that’s wrong. I just checked the scan I have of my form, you can list side effects in the part where it asks you what medicines you take. (Page 5 “About your medication”)
    The section they describe here really is to do with whether or not you think your side effects are due to drug *misuse*. (Page 6 “drugs alcohol and other substances).

    Tabbii is correct here. There are other places you can mention your side effects. If the “about your medication” bit is filled up just listing meds (like mine was) just write it in the box “other information” for anything else you want to tell them or write on another bit of paper – just remember to pop your name & national insurance number on the top of the sheet so it doesn’t get lost.
